Welcome to Lumiere Medspa! These Terms of Service ("Terms") govern your use of our services, including all treatments, procedures, and products offered at our medspa, located at 89 S Evergreen Ave, Studio 28, Arlington Heights, IL, 60005, as well as our website, social media, and other communication channels. By booking an appointment or purchasing any services or products from us, you agree to the following terms and conditions.
1. General Terms
1.1. Eligibility: Our services are available to individuals aged 18 years or older. By scheduling an appointment, you confirm that you meet this age requirement.
1.2. Medical Information: You are responsible for providing accurate and up-to-date medical history before receiving any treatments. This includes information on allergies, medications, and pre-existing conditions.
1.3. Consultations: All new clients must undergo a consultation to assess their needs, medical history, and eligibility for services.
1.4. Results: Individual results may vary based on several factors, including skin type, health conditions, and adherence to post-treatment care instructions.
2. Appointment Booking and Cancellation
2.1. Booking: Appointments can be booked through our website, over the phone, or in person. A valid credit card is required to secure your appointment.
2.2. Cancellation Policy: We require at least 24 hours' notice for any cancellations or rescheduling. Failure to cancel within this time frame will result in a cancellation fee of $100.
2.3. No-Shows: Clients who do not show up for their appointment without prior notice will be charged 100% of the scheduled service.
2.4. Late Arrivals: If you arrive more than 15 minutes late, your appointment may be shortened or rescheduled. Late arrivals may also incur a fee.
3. Payments and Refunds
3.1. Payment Methods: We accept major credit cards, debit cards, and cash. Full payment is due at the time of service.
3.2. Package Deals and Promotions: All package deals, including multi-session treatments, must be paid in full upfront. Promotions and discounts are subject to availability and cannot be combined with other offers.
3.3. Refunds: All sales are final. Due to the nature of our services, we do not offer refunds on treatments or products once purchased. In the event of an adverse reaction or dissatisfaction, a complimentary consultation will be provided to assess the next steps.
4. Gift Cards and Vouchers
4.1. Gift Cards: Gift cards purchased at Lumiere Medspa are non-refundable and must be used within 6 months of purchase. Gift cards cannot be redeemed for cash.
4.2. Promotional Vouchers: Promotional vouchers, including those won in giveaways, are subject to expiration dates and may be limited to specific treatments.
5. Privacy Policy
5.1. Personal Information: Your privacy is important to us. We collect personal and medical information solely for the purpose of providing treatments. We do not share or sell your information to third parties without your consent, except as required by law.
5.2. Marketing Communications: By providing your email address or phone number, you agree to receive marketing and promotional messages from Lumiere Medspa. You can opt out at any time by following the unsubscribe instructions in our communications.
6. Medical Disclaimer
6.1. Not Medical Advice: Lumiere Medspa provides aesthetic and wellness services. These services are not a substitute for medical advice, diagnosis, or treatment from a healthcare professional.
6.2. Risks: All treatments carry potential risks. It is your responsibility to follow pre- and post-care instructions provided by our practitioners to minimize risks and enhance results.
7. Limitation of Liability
Lumiere Medspa is not liable for any injuries, adverse reactions, or other complications resulting from treatments that have been provided with your informed consent. In no event shall our liability exceed the amount paid for the treatment in question.
8. Right to Refuse Service
Lumiere Medspa reserves the right to refuse service to any client for any reason, including but not limited to inappropriate behavior, failure to follow medical advice, or non-compliance with these Terms of Service.
9. Changes to Terms
We reserve the right to modify or update these Terms at any time. Any changes will be posted on our website and will take effect immediately.
